TEAM LEGACY DOJO (TLD)

WARNING, WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK

This agreement must be signed by all persons who wish to participate in any Team Legacy Dojo martial arts class, event, or activity.

In consideration of being allowed to participate in any way in the classes, events, and/or activities of Team Legacy Dojo:

1. I recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability, death, or economic loss due to injury.
   
   

2. I understand that such risks may arise from my own actions or inactions, the actions or inactions of others, the rules of participation, the condition of the premises, or the use of equipment.
   
   

3. I recognize that there may be other risks not known or reasonably foreseeable at this time.
   
   

4. I agree to inspect the facilities, equipment, and pairings prior to participation. I will immediately inform an instructor if I believe anything is unsafe or beyond my capability and will refuse to participate if so.
   
   

5. I voluntarily assume all of the foregoing risks and accept personal responsibility for any damages resulting from injury, permanent disability, or death.
   
   

6. I participate in martial arts training and/or competition entirely of my own free will and understand the importance of following all rules and safety guidelines.
   
   

7. I certify that I am in good physical condition and have no medical condition that would impair my ability to safely participate in training or competition.
   
   

8. I understand that photos or videos of myself or my child may be taken and used for promotional purposes by Team Legacy Dojo, staff, parents, or other participants. If I wish to opt out, I will underline this section, initial, and date it.
   
   

9. I release, waive, discharge, and covenant not to sue Team Legacy Dojo and its affiliates, instructors, personnel, property owners, and all associated parties from any liability for injuries (including death) or property damage arising out of participation, even if caused by negligence.
   
   

10. I understand that Team Legacy Dojo is a martial arts training facility and not a licensed daycare provider and does not offer related childcare services.
    
    

I HAVE READ AND FULLY UNDERSTAND THIS WAIVER. I SIGN IT VOLUNTARILY AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.

SAFETY ACKNOWLEDGEMENT — LIABILITY WAIVER AND RELEASE OF CLAIMS

Duty to Self-Monitor:
Participants agree to monitor for any signs of illness. Anyone experiencing symptoms like fever, cough, or fatigue agrees not to attend until fully recovered.

Waiver and Assumption of Risk:
I willingly engage in TLD activities and acknowledge and assume all risks, including exposure to infectious diseases or injury. I release and waive all claims against Team Legacy Dojo and its affiliated parties.

1. Participation may involve exposure to infectious diseases.
   
   

2. I freely assume such risks.
   
   

3. I accept responsibility for any injury or harm during the activity.
   
   

4. TLD may delay membership benefits at its discretion.
   
   

I HAVE READ AND FULLY UNDERSTAND THIS WAIVER. I SIGN IT VOLUNTARILY AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.

MEDICAL ACKNOWLEDGEMENT AND RELEASE

I acknowledge that participation may involve health risks, including dizziness, fainting, injury, or even death. I will stop immediately and seek help if I feel unwell.

I RELEASE THE RELEASED PARTIES FROM LIABILITY FOR ANY MEDICAL CARE RENDERED IN CONNECTION WITH MY PARTICIPATION.

I assume all risks associated with attending or participating in TLD activities and release all involved parties from responsibility for injury, death, or property loss.

PAYMENT POLICY

By signing this form, I agree to the following:

• At sign-up, I will pay the first and last month’s fees upfront.
  
  

• After that, membership continues month-to-month and is billed automatically.
  
  

• Team Legacy Dojo does not offer account freezes during summer breaks, but you may choose from the following two options:
  
  Option 1: Pay a flat monthly "freeze fee" of $79 per member during your time away to maintain your account and return at the original payment rate.
  
  Option 2: Cancel your membership. No further payments will be charged. Students are allowed to train for 30 more days from cancellation day or biling day, whichever comes first. However, when you return, you will need to re-enroll and pay the first and last month’s fees again at the current rate. 
  
  

• All payments are securely processed and visible in the Gymdesk Member App.
  
  

CANCELLATION POLICY

If you choose to cancel your membership:

• You must notify us via email.
  
  

• Your billing will stop immediately following the end of your current paid month.
  
  

• Please note that the initial last-month payment will be applied to cover your final month of training.
  
  

I HAVE READ AND UNDERSTAND ALL TERMS IN THIS DOCUMENT. I VOLUNTARILY SIGN IT WITHOUT ANY INDUCEMENT OR GUARANTEE AND ACKNOWLEDGE THAT I AM GIVING UP SIGNIFICANT LEGAL RIGHTS.
